Persons entitled to benefit from the MultiSport cards are employees employed at Jagiellonian University Medical College and additional persons registered by the employee: accompanying person, children, teenagers, seniors. The program makes it possible to use the services of sports and recreational facilities in Cracow and the whole of Poland.

Those interested in participating in the Program are asked to fill out and deliver the appropriate forms to the PAC JU MC Social Affairs Team:

1) an application form for a MultiSport card for an employee and, an Employee – consent form for Benefit Systems S.A. to process personal data. – applying for a card for themselves
2) the application form for an additional person for MultiSport Card and, respectively, the form for:

MultiSport Card for Accompanying Person
MultiSport Kids Card – child under 15 years of age
MultiSport Kids Aqua Card – child under 15 years of age
MultiSport Student Card – underage child between 16 and 18 years of age
MultiSport Student Card – adult child between 18 and 26 years of age
Senior Card – senior over 60 years of age

A set of forms must be delivered by the 10th of the month preceding the month from which the card is to be activated.

Cancellation of participation in the Program must be submitted in writing by the 10th day of the month preceding the month from which the cancellation takes place, and the MultiSport card must be returned to the Social Affairs Team of PAC JU MC by the 5th day of the month in which the employee resigns.
