Persons entitled to use MultiSport cards are employees employed at Jagiellonian University – Collegium Medicum and additional persons registered by the employee: accompanying person, children, teenagers, seniors. The program makes it possible to use the services of sports and recreational facilities in Cracow and the whole of Poland.

Those interested in participating in the program should fill out and deliver the appropriate forms to the Social Affairs Team. An application form for the employee, an application form for an additional person, and a consent form for Benefit Systems S.A. are available in the Forms Library.

Cancellation of participation in the Program must be submitted in writing by the 10th day of the month preceding the month of cancellation, and the MultiSport card must be returned to the Social Affairs Team MC by the 5th day of the month from which the employee resigns.
